Utilizing goal forms

Goal forms can be utilized for writing student goals and strategies.
depending on goal sheet format.
Coding system ex.
LA- Language Arts,
R- Reading.
M- Math,
SS- Social Studies,
SC- Science
for grades lower than average (ex. like a C):
use an asterisk * for a D,
use double asterisks ** for non-passing grades or letters
see example below:
STUDENT #2 Report card grades
LA= C
R= B
M= *
SS= **
SC= A
- Students can place initials on top of form.
- Goals and strategies can be listed according to chosen format
- Students sign and date their copies when reviewing
depending on goal sheet format.
- Teacher or staff member can also keep a copy of form to be
